Assigned to Entrada Home Visiting Program

Full-time (37.5 hours)

Primarily Monday – Friday 9a-5p with flexibility to work one evening per week

 

Qualifications:

  • Associates Degree required, Bachelors’ preferred in early childhood or related field such as social work, human service or community health.
  • Required prior experience with young children and parents; home visiting experience preferred
  • Certified Parents as Teachers home visitor preferred with both foundational and model implementation training elements
  • Certification in Domestic Violence (minimum 25 hours) preferred
  • Must have knowledge of resources within the community
  • Must have excellent written and organizational skills including computer skills
  • Must be committed to agency mission and be able to actively and responsibly support its vision in a positive manner within the agency as well as throughout the community
  • Must have positive energy, sound judgment, ability to work independently as well as part of a team and be able to work effectively in partnership with other agency staff to the project
  • Reliable vehicle, valid driver’s license and telephone required
  • Must be able to lift 40 lbs.
  • Bilingual/Bicultural (Spanish) preferred

 

Duties and Responsibilities: 

  • Conduct intensive home visiting to pregnant and parenting families up to age 27 living in Springfield, prenatally to Kindergarten as part of the Entrada Home Visiting Program; a team consisting of Early Head Start, Healthy Families and Parents as Teachers funded by DPH under the Maternal Infant and Early Childhood Home Visiting Program (MIECHV) and known as the MA Home Visiting Program
  • Serve 25 families in the first year, increasing # of families after completion of year 1
  • Keep fidelity to the Parents as Teachers model by adhering to all program elements and requirements
  • Facilitate or co-facilitate group connections with the Entrada team
  • Receive and respond to all referrals that come from Square One’s Assistant Coordinator
  • Meet specified federal benchmarks through the use of required tools and screening mechanisms including but not limited to ASQ-3, ASQ-SE, Edinburgh maternal depression scale, SBIRT and life skills progression
  • Conduct all intakes, assessments and individualized goal plans within deadline
  • Conduct domestic violence assessments with all parents identified at being at risk for or  involved in unhealthy relationships including the development of a safety plan and linkage with ongoing supportive services and necessary resources to enhance safety
  • Provide consultation to the Entrada team regarding any domestic violence related concerns
  • Crisis intervention as necessary
  • Referral and follow-up to necessary resources
  • Participate in Entrada Advisory Council meetings
  • Assist parents in working toward self-sufficiency
  • Coordination and networking with other services and resources available
  • Ensure that participant files are complete and properly maintained
  • Enter all data into PDS (Participant Data System) and Client Track within deadline
  • Attend all required staff meetings and trainings (may require travel)
  • Provide transportation to participants within a self-sufficiency model, ensuring that participant is designing a plan to secure own future transportation after realistic time frame;
  • Adhere to all safety laws/policies regarding seat belts, car seat requirements and speed limits at all times.
  • Complete yearly Parents as Teachers recertification, ensuring that all training requirements have been met
  • Perform other related duties as needed

 

 

Location:  Springfield                    Hourly Rate:  $12.82
